Forms
The Written Submission Form is used by rights holders, landowners and the general public to document concerns to the Commission about proposed oil and gas activities.
The Written Report Cover Sheet must be submitted with a written report of consultation and notification activities.
A Written Report Line List of consultation and notification recipients must be submitted with the written report.
