Forms
The Corporate Structure Change Application Form is used by permit holders to amalgamate corporations, change corporate names or change corporate structure.

The Geophysical Permit Transfer Application Form is used by permit holders to transfer ownership of a geophysical permit.
The New Company Application Form is used by applicants to become a registered company with the Commission. Permit applications will not be accepted by the Commission unless the applicant is registered.
The PIMS Facility Transfer Application Form is used by permit holders to transfer ownership of a PIMS facility.
The Pipeline Project Transfer Application Form is used by permit holders to transfer ownership of a pipeline project.
The Road Permit Application Transfer Form must be submitted to the Commission by the applicant when transferring a Road Permit.

A Schedule of Geophysical Permits is to be attached to the Geophysical Permit Transfer Application if required.

A Schedule of Pipeline Projects Permits is to be attached to the Pipeline Project Transfer Application if required.
A Schedule of Site Detail Permits is to be attached to the Site Detail Transfer Application if required.
A Schedule of Well Permit & Authorizations is to be attached to the Well Permit & Authorization Application if required.
The Site Detail Transfer Application Form is used by permit holders to transfer ownership of a site detail.
The Well Permit & Authorization Transfer Application Form is used by permit holders to transfer ownership of a well permit.
