Forms
The Annual Gas Overproduction Report allows industry to report overproduction of gas from a well producing under a daily gas allowable.
The BC S18 Monthly Injection Disposal Statement Form is required for reporting monthly injection and/or disposal of gas or liquid into a well.
The BC-11: Notice of Commencement of Suspension of Operations Form is required for initiating or suspending any and all operations on a well. The form can be found at the following link:

The Daily Gas Allowable Application Form allows industry to request a daily gas allowable amount.
The Daily Oil Allowable Application Form (Project) allows industry to request a single oil allowable for oil wells producing from an approved reservoir project.
The Daily Oil Allowable Permit Amendment Application Form allows industry to apply for an amended amount of daily oil allowable when the previously applied for amount exceeds the TVD calculated value.
The Notice of Commingled Production Form allows industry to notify the Commission regarding any commingled well production.
The Production Allowable Report – Crude Oil allows industry to report monthly oil and gas volumes, GOR adjusted monthly oil allowable volume and over/under oil production value.
The Reservoir Pressure Survey Test Report Form is a summary report, attach to submitted data. Instructions on how to complete the form are also included.

The Summary Information: Commingled Production provides information regarding commingled production. Commingled production is the flow of fluids, originating from two or more pools, in an unsegregated manner to a well measurement meter.
