The Engineering Data Sheet for Re-Entry must be submitted with an application to re-enter a well.
The Notice of Well Construction Starts is used by permit holders to notify the Commission of well construction. It must be submitted 48 hours before construction begins.
The Subsequent Well Application Form (with no new area) is used by permit holders to apply for a subsequent well with no new area either after the primary well has been applied for or with a multi-well package.
