The Environmental Site Assessment Checklist must be completed and submitted as part of an Environmental Site Assesment Report submitted under Section 7(b) of the Dormancy and Shutdown Regulation and as part of an application for a CoR under Section 41(1) of the Oil and Gas Activities Act.

The Site Classification Form allows industry to demonstrate site condition and regulatory authority responsible for oversight of remediation to the Commission.
The Access to Crown Land for Offsite Investigation Form is used by permit holders to apply for Crown land access for the purpose of environmental investigation related to existing oil and gas activities.
The Schedule B – Site Reclamation Assessment provides industry with the reclamation assessment requirements for lands within the Agricultural Land Reserve.
The Wellsite Cleanup Form is required after a well has been drilled.
Part 1 of the Certificate of Restoration Application Form is used by permit holders to apply for a remediated site status.
Part 2 of the Certificate of Restoration Application Form is used by permit holders to apply for a reclaimed site status.
