The Form D - Permit Holder Post Incident Report is to be submitted by the permit holder within 60 days following a level 1, 2 or 3 emergency, any pipeline incident, or upon request from the Regulator.

Form M1 - Notification of a Full-Scale Exercise for a Facility/Pipeline Start-Up/Modification
A full-scale exercise is required prior to start-up for any of the following: A processing facility, an LNG facility, a petroleum or natural gas pipeline that is not part of a gathering system or fuel gas system, or any other facility, pipeline or equipment that is specified in EMR 4.1(3). The notification must be sent to emp@bc-er.ca. Minimum 30 days' notice is required.

Form M2 - Notification of ERP Review Meeting
The drilling crew and company representatives on a drilling site must be familiarized with the Drilling and Completions ERP. Prior to drilling operations, an ERP Review Meeting is required before surface casing is set for the first well on any multi-pad well. If the rig crew for subsequent wells on a specific pad site is different, or if a new drilling and completions ERP has been submitted for a well or well pad site, a subsequent meeting is required. The notification must be sent by email to emp@bc-er.ca. Minimum 2 business days' notice is required.
